ABSTRACT

Metformin is recommended as initial monotherapy for treatment of type 2 diabetes mellitus because it decreases the higher blood glucose by suppressing hepatic production of glucose, apart from suppression of hepatic glucose production, it also increases sensitivity of insulin, it also enhances the peripheral uptake of glucose (by inducing GLUT4 enhancer factor phosphorylation), and it also decreases the insulin-induced suppression of fatty acid oxidation. The aim of this study, metformin and combination of metformin and sitagliptin in type ii diabetic mellitus patients. Methods: Two groups were included in this study. Each group has 50 cases & each case was having diabetic mellitus. This study conducted in the Career Institute of Medical Sciences in the Department of Pharmacology. The duration of study was over a period of six month. Results: In our study we were included two groups. Each group has 50 cases, means total 100 cases were included. In group I we were observed 26 male & 24 female out of 50 cases. In group I we found that 13 had vomiting followed by diarrhea, metallic taste, abdominal pain. While in group II we found that 3 had vomiting followed by diarrhea, metallic taste, abdominal pain. Conclusion: The foregone discussion revealed that in patients who are on monotherapy with metformin alone having inadequate glycaemic control. The addition of one daily dose of Sitagliptin 100 mg is the most effective way of maintaining glycaemic control.

ABSTRACT

National Guidelines on Infant and Young Child Feeding should become an integral part of nation-wide Integrated child development Services (ICDS) and the Reproductive and Child Health (RCH) Programme. These need to be effectively operationalized through the programme managers and field functionaries of these on-going programmes. This can happen only when they are practically oriented to the correct norms of IYCF. Methods: A cross sectional study of complementary feeding practices amongst mothers of children aged six months to two years in an urban setting. The place of study is an immunization clinic in a tertiary care hospital of a medical college located in urban area. Study population comprised of mothers of children aged six months to two years attending an immunization clinic. Study was approved by college Institutional Ethics Committee. Mothers who met the inclusion criteria, attending the immunization clinic, from the selected tertiary care hospital of a medical college were selected by systematic random sampling and interviewed. Data analysis done with the using Microsoft excel. Results: Out of 154 mothers, 51.3% of the mothers were in the age group of 25-34 years followed by 47.4% of the mothers in the age group less than 25 years and only 1.3 % of the mothers were more than 35 years of age. Mean age ± SD of the mothers was 25.26 ± 3.73 with the youngest and the oldest mother being 19 and 39 years old respectively. Among the respondents by religion, majority (80.5%) were Hindus followed by 15.6 % Muslims and only 3.9% were Christians. Conclusion: Time of complementary feeding initiation varied according child’s gender, predominant caregiver of the child, mother’s parity and socio economic status of the family, which were found to be statistically significant.

ABSTRACT

Alkaline phosphatase, ALP is found in all tissue of the body.(4) The enzyme responsible for extracellular catabolism of glutathione’s is Gamma glutamyltransferase, GGT]. It is the main thiol intracellular antioxidant agent. The larger function of enzyme is to transport amino acid and peptide into the cell across the cell membrane in the form of gamma glutamyl peptidase. The only iodine containing amine hormones in the vertebrate are Triiodothyronine (T3) and thyroxine (T4). Methods: All samples were taken from hypothyroid, hyperthyroid and euthyroid subjects who fasted for at least 8 hours before the blood collection.Serum separated and analyzed for T3, T4, TSH, CK-NAC, CK-MB, LDH and AST(SGOT) . The sample was processed in standardized manner under set protocol in the Department of Biochemistry. Results: Among the total of 300 subjects of ages 25 -50 years of both sexes were included in the study, 100 were hypothyroid, 100 were hyperthyroid and 100 were euthyroid. Their serum levels of T3, T4 and TSH were estimated and confirmed. Conclusion: The present study might be beneficial in public health and clinical practice, as both thyroid disorders and cardiovascular diseases are important issue of concern to reduce the morbidity and mortality.

ABSTRACT

Introduction: In addition to smoking, smokeless tobacco hasalso evidently shown its effect on various oral tissues.Smokeless tobacco products have been in existence forthousands of years among populations in South America andSoutheast Asia in varied forms.Methodology: This cross-sectional study investigated theeffect of commonly used smokeless tobacco on periodontalhealth in local population of nearby villages of Air Force StationBakshi Ka Talab, U.P., India, for which 2045 individuals[males=1455 (71%), females=590 (28.9%)] were evaluated.Results: Maximum incidence of smokeless tobaccoconsumption was observed in the age group 21–30 years(39.4%), whereas minimum consumption was observed in theage group 10–20 years (7.5%). The peak smokeless tobaccoconsumption was observed between the ages 21 and 40 years.Recording of oral hygiene measures included in this studyshowed majority of subjects (n=1961) used tooth brushing asthe oral hygiene measure, while some used datoon (n=30;1.5%) for cleaning their teeth and others used their finger(n=54; 2.6%) with or without tooth powder.Conclusion: This study conclude that, PPD were significantlyhigher statistically amongst longer duration (>5 years)smokeless tobacco users as compared to non-tobacco users.

ABSTRACT

Introduction: Infant and young child feeding indicators havebeen developed by the World Health Organization (WHO), toguide and monitor the feeding practices of young children. TheWorld Health Organization (WHO) recommends exclusivebreast feeding (EBF) for the first six months of life, with theaddition of complementary feeds (CF) at six months withcontinued breast feeding until at least the age of two years.Study aimed to find out the complementary feeding practicesamongst mothers of children aged six months to two years.Material and Methods: A cross sectional study ofcomplementary feeding practices amongst mothers of childrenaged six months to two years in an urban setting. Therefore atotal of 154 subjects were taken into the study. The study wasconducted from Jan 2014 – Sep 2015.Result: Among the studied children, 59.7% were males and40.3% were females. Almost more than half of the childrenwere in the age groups of more than 9 months and only 18.2%were in the age group 6 – 8 months.Conclusion: This study suggested that accurate informationand education should be given to mothers and caregiversabout appropriate breast-feeding and complementary feedingpractices to prevent malnutrition and improve the health statusof the children

ABSTRACT

Serum uric acid (SUA) is an end product of purine metabolism. SUA is excreted mainly through the kidneys. Multiple mechanisms have been reported to show the association of hyperuricemia with glucose intolerance. The association between insulin and renal resistance to absorption of urates has been found out to be the most important among these mechanisms. Methods: This comparative study contains 50 subjects of type 2 diabetes having an age group of 40-65 years were participated in case & 50 healthy subjects as control group. This case control study conducted in the department of department of biochemistry, LPS institute of Cardiology, GSVM medical college, Kanpur. Results: In the case-control study were found 54% male and 46% female in case group & 42% female and 58% female. We suggested that in study group 44% cases were belongs to 61-65 age group followed by other while in control group 42% were belongs to 61-65 age group followed by other age group. Conclusion: This study conclude that there were significant correlation between uric acid and HOMA-IR value. Hyperuricemia is related to risk factors for insulin resistance in type 2 diabetes. The study concluded a significant progressive relationship between increased uric acid level with respect to HOMA-IR levels which is an indices of insulin resistance.

ABSTRACT

To measure QoL outcome in post LASIK Aviators using the Quality of Life Impact of Refractive Correction (QIRC) questionnaire. Methods: A prospective, non-randomized, cross-sectional study was carried out from Oct 2016 to Dec 2017. . The study included actively flying Indian pilots of both sexes, reporting for the annual medical fitness certification. QIRC which consists of 20 questions scored on 5 category response scale, is self-administered. Aviators were asked to rate on 5 point scale of agreement. Response was converted to Rasch weighted scores to give a true measure of refractive error related QoL. Results: This hospital based study was conducted on 39 Pilots who were administered Quality of Life Impact of Refractive Correction (QIRC) questionnaire. 20 pilots were from civil airline pilots and 7 were Army Aviation and 2 were IAF pilot of transport stream. There were 3 female pilots all from the civil transport stream. Mean age 27.2 yrs ± 4.5 (21 to 37 yrs) who had myopia of more than- 0.50 D, ranging from -.75D TO 6.5 D before undergoing the refractive surgery procedure. Prior to surgery predominantly i.e.26 patients (66.6%) were spectacle wearer, 10 (25.6%) were contact lens wearer, 3 patients (7.69%) were not using any means of correction. All the patients were Post Lasik status of more than 1 year. This study showed a large improvement in QoL in the majority of patients after refractive surgery (PRK & LASIK). These large improvements are consistent with results in conventionally validated previous reports of refractive error related QoL as a result of surgery. In the POST LASIK study group spectacle correction was being used by 36% of patients with power ranging from –0.50D to – 1.50 D SE. Conclusion: LASIK is an excellent alternative to glasses or contact lens use, being associated with high degree of patient’s satisfaction. While LASIK is not without risks, these results are quite reassuring for all pilots thinking of this procedure. Laser system for vision correction like aircraft are continually being enhanced, improved and upgraded. We should embrace the technology and offer the best currently available to the aviators.

ABSTRACT

Background: The cornea is an important ocular structure involved in the mediation of visual perception. It is the principal refractive surface of the eye and vision can be significantly affected by relatively small changes in its structure and parameters. Measurement of corneal parameters is important in the diagnosis and management of ocular diseases such as keratoconus and glaucoma Methods: This study was conducted in the department of ophthalmology in ACMS medical college Delhi. Results: This study showed, height and AL has a positive correlation with a value of +.119, and with weight AL has a weak positive correlation (+.035). CCT has a positive correlation with age (+0.044) but was found to have nil correlation with sex. Conclusion: This study was conclude that, no significant difference in mean CCT and AL was found between male and female subjects.

Background: Acne vulgaris is a pilosebaceous gland disease usually affecting people from puberty to young adulthood due to hormonal changes, but the exact relationship of various hormone to acne is not known. Aims:- The present study has been planned to find out the association of hormonal status with the different grades of acne vulgaris female patients.. Methods: Fifty patients each of Mild (group I), Moderate (group II) and Severe Acne Vulgaris (group III) in age group 15-40 years were included as cases. The fasting blood samples were collected on the second day of menstrual cycle of the patient and before the start of treatment. Written and informed consent was taken from all subjects in the cases and control groups after explaining about the details of the study. Results: Levels of Luteinizing Hormone (LH), Estrogen, Testosterone, Insulin, Follicle Stimulating Hormone (FSH) and Prolactin in different grade of acne vulgaris patients were compared with control groups. Statistical analysis was done using Kruskal Wallis test and ANOVA. Conclusion: Level of LH, Estradiol,Testosterone andInsulin may be studied in cases resistant to conventional treatment of acne in women while the levels of FSH and prolactin, need not to be studied.

ABSTRACT

Background: The uric acid in serum (SUA) is significantly related with cardiovascular disease, it is proved by clinically. The increased level of uric acid in the serum lead to highly platelet reactivity mediating inflammation and stimulate of smooth muscle cell production which probably degrades acute thrombosis. Methods: The blood samples were collected and sent to biochemistry laboratory for estimation of uric acid. This study conducted in the Departments of Biochemistry, Dr. Ram Manohar Lohia Medical College Lucknow. Results: In this study 200 patients included with acute myocardial infarction. Among the 200 patients, 148 (74%) were male and 52 (26%) were female. Among the 25-70 Age group , most of people from 56-70 age group. Mean of uric acid level in male cases was 5.6 and female cases was 5.2 , male controls was 4.2 and female controls was 3.6. Conclusion: The uric acid level in the serum are high in patients which had acute myocardial infarction as compare to other persons. It is also higher with the killips classes. The Killips classification and uric acid level in the serum is a good analyst of mortality after acute myocardial infarction, if is combined.

ABSTRACT

Background: Age-related macular degeneration (AMD) is the leading cause of irreversible blindness. Currently 2.5-3 million people worldwide are blind due to AMD. Objective:- This study were to  use of anti-angiogenic drug in the treatment for age related macular degeneration. Methods: A cross sectional study was carried out among patients attending Eye OPD of a tertiary care eye center. Results: 25 patients were included in this study . Among the 25 patients 15 were male & 10 were female. Mean visual acuity (VA) in this study improved from 5/60 to 6/36 during follow up. This was statistically significant (P <0.0001). Conclusion: The results of this study suggest that IVB (1.25 mg) is well tolerated and associated with stabilization or improvement in visual acuity, decreased central macular thickness (CMT) by ocular coherence tomography (OCT), and reduction in angiographic leakage associated with neovascular AMD.

Background: Vitamin D deficiency is recognized as a global public health problem. In India Vitamin D Deficiency is prevalent, a finding that is unexpected in a tropical country with abundant sunshine. This study evaluated prevalence of vitamin D deficiency among geriatric patients.Methods: The present study was a hospitalized based cross sectional study conducted among geriatric patients attending the ortho. OPD of CIMS, Lucknow. Those elderly (>60 yrs) consenting to participate were including in this study .Patients who were on vitamin D3 & calcium supply (6 month) were excluded. Results: In this study we have taken 200 sample size. Geriatric patients who were not on vitamin D and calcium supplements for last 6 months were included for the study. Out of 200 participants, only 27(13.5%) participants had normal vitamin D levels and 173 (86.5%) study participants had low vitamin D levels among them 112 (56%) participants had vitamin D deficiency, 61(30.5%) participants had vitamin D insufficiency. Conclusions: Present study confirmed that Vitamin D deficiency is a major public health problem among geriatric age group.

ABSTRACT

Background: Free radicals and other reactive species are constantly generated in vivo and cause oxidative damage to biomolecules. DNA is probably the most biologically significant target of oxidative attack. Among numerous types of oxidative DNA damage the formation of 8-hydroxyguanosine (8-OHdG) is a sensitive biomarker of oxidative stress, an adduct formed as a result of biochemical reaction between ROS and DNA. Chronic exposure to Organophosphorus (OP) pesticides is implicated in many health conditions that result from the induction of oxidative stress, including cytogenetic damage. The main objective of the study was to evaluate the biochemical levels of 8-OHdG in spot urinary samples under the exposed OP pesticide sprayers and farm workers. Methods: In this study, 51 male pesticide sprayers and 39 farm workers in the age group of 18-47 years having exposure ranged from 3 to 15 years in duration were selected. The referents (n=31) were selected on the same criteria as well as they were never exposed to pesticides at any time. This study was conducted during the growing season (January, 2009 – September, 2010). The most commonly used OP pesticides like chlorpyriphos, Diazinon, Dimethioate, Monocrotofos etc., were used in this study. Urine samples from each participant were taken in sterile tubes and were stored at -200C till analysed. The concentration of 8-OHdG in samples were analyzed using ELISA. Results: The urinary levels of 8-OHdG were found to be significantly higher in the farm workers and pesticide sprayers in contrast to the level observed in the control group (p<0.05). When the data was analyzed in the exposed groups in relation to duration of exposure it was found that both the farm workers and sprayers who were exposed to OP pesticides for less than 5 years showed the maximum mean values of 8-OHdG in comparison to those exposed to for more than 10 years. Conclusions: In view of this regular bio monitoring studies in target human populations are imperative necessary due to frequent changes in pesticide formulations and introduction of newer pesticides. Despite that several life style factors may influence the urinary concentrations of 8-OHdG but still this non-invasive bio-marker 8-OHdG is preferred over other invasive techniques to evaluate the environmental and occupational exposure effect of OP pesticides on the genotoxicity of the exposed workers.
